Position Responsibilities:
- Review and verify medication orders with accuracy and clinical judgment
- Collaborate with healthcare providers and long-term care facility staff
- Provide medication therapy recommendations and support
- Ensure compliance with pharmacy regulations and best practices
- Promote safe medication use and positive resident outcomes
- Participate in quality improvement initiatives
Qualifications:
- Active Pharmacist license (or ability to obtain Iowa licensure)
- PharmD or equivalent pharmacy degree
- Strong attention to detail and commitment to patient care
- Excellent communication and collaboration skills
- Long-term care, consultant pharmacy, or clinical pharmacy experience preferred (but not required)
